At Grandbrook Home Care Facility, we are an APD-approved, behavioral-focused group home for individuals with developmental disabilities. We operate two co-ed group homes: Braden River Group Home, serving individuals 18 and older, and Mahogany Group Home, serving individuals 22 and under. Our mission is to provide compassionate, structured care that promotes independence, stability, and an enhanced quality of life.

Encouragement of social engagement and participation in community activities.
Behavioral Support
Implementation of individualized behavior plans to promote positive outcomes.
Care Coordination
Ongoing collaboration with APD, support coordinators, and clinical providers.
